don't really know, in win98 there were a .shelliconcache file or so in the win folder, just delete this.
win xp recognizes if a folder contain images and set the properties for this folder. Go to explorer view>folder customize or so (i have a german win) and change the folder type to "documents".

I also have this problem, overwrite a folder icon with the same name but other content, windows will need a while to change the icon... there must be a commandline like this rundll.exe ......, but i don't know...
