I've now received responses from all but two theme authors. The following

work, your approval and your swift responses.

I'm not entirely sure how to best provide credits, but I think we should
see about moving the relicensed themes out of the main Rockbox
distribution, and instead distribute them through the theme site.

Keeping them in SVN seems like it would be somewhat useful though.

This leaves us with only two authors, and as many themes that we can't
relicense yet:
  - marquee (GPL), by Mike Sobel
  - zezayer (GPL), by Jake Owen (zezayer)

Both of these are only included on h100 and the greyscale ipods I believe.
  If it turns out we don't hear from Mike or Jake, I think we should just
drop their themes entirely, or put them in a graveyard on the wiki.
