A couple of weeks ago, I ran into a mysterious "create failed" error
when creating a bookmark, but I couldn't reproduce it. Now I ran into
the same problem, and noticed the following settings:

  General Settings
    Bookmarking
      Bookmark on Stop: Ask - Recent only
      Load Last Bookmark: Ask
      Maintain a list of Bookmarks: No

When changing "Maintain a list of Bookmarks" to "Unique", the problem
disappears.
