FS#1374 - create_xing_header() produces bad output for large files

The method create_xing_header() uses to generate TOC indices
is flawed; it shifts off the 8 MSBits of the fileposition. This is fine for
most files, but when the fileposition is greater than 16,777,215, it
starts wrapping to zero – preventing anyone from seeking past that
point in a VBRFixed file.
